At Berks Plastic Surgery, board-certified plastic surgeon Dr. Brian Reedy is dedicated to offering breast augmentation patients the safest, most advanced options on the market—so when the Motiva® SmoothSilk Round and SmoothSilk Ergonomix® implants became available in the US, we made sure to have them on hand right away. Patients love the results of Motiva implants, and they are quickly becoming the implant of choice at our practice. Below, Dr. Reedy answers the questions patients have been asking since Motiva arrived.

Do Motiva implants move more naturally with the body?

Yes, especially the Ergonomix line. Because the gel inside is progressively viscoelastic, it flows with gravity and motion, so the implant responds like natural breast tissue when you jog, stretch, and yes—during intimate moments. Patients report less “implant awareness” during vigorous activity compared with firmer traditional gels, and partners often can’t distinguish the implant from natural breast tissue by touch.

What is the difference between Ergonomix and Round? Which style is better if I have very little breast tissue?

Motiva offers two implant shapes:

  • SmoothSilk Round – Retains a round profile in every position for consistent upper-pole fullness (great if you want a “push-up-bra” look).
  • SmoothSilk Ergonomix – Contains a highly viscoelastic gel that redistributes with gravity, so the implant looks round when you’re lying down and gently teardrops when you stand for a more natural drape.

Patients with very thin tissue often prefer Ergonomix because the softer gel and shape-shifting profile better mimic natural breast movement and minimize any visible edge.

Can I get Motiva if I have a low BMI?

Absolutely! Low-BMI patients are often ideal candidates. The gel in both Motiva implant options provides soft coverage even when breast tissue is minimal, and the implant shell is designed to resist rippling (wrinkles sometimes seen in earlier breast implant options on thinner patients). Dr. Reedy carefully evaluates skin quality and chest width to pick the safest size and projection to achieve the results you’re looking for.

How is Motiva different from Natrelle?

Allergan Natrelle® offers multiple gel cohesivities, classic smooth shells, and a vast size range. Motiva’s differentiators include:

  • Next-gen shell: Motiva’s micro-textured SmoothSilk surface is engineered to reduce bacterial adhesion compared with classic smooth shells.
  • Dynamic gel: Motiva Ergonomix gel flows more freely, so the implant rounds out when you’re lying down and drops into a teardrop when you stand.
  • Warranty: Both brands have robust warranties, but Motiva bundles its BluSeal integrity check, optional Q-ID chip, and an optional extended warranty, which some patients find reassuring.

If dynamic movement and enhanced safety technology appeal to you, Motiva may edge out Natrelle. If you prefer a more time-tested implant, or you already have Natrelle implants you love, Natrelle remains an excellent choice. We are still very pleased with the safety and performance of Natrelle implants and will continue to offer them along with Motiva implants.

How does Motiva’s SmoothSilk surface lower my risk of capsular contracture?

Capsular contracture is triggered in part by bacteria and inflammation around the implant. Motiva’s SmoothSilk shell has a 4-micron “nano-smooth” topography—too fine for bacteria to anchor easily, yet not textured—helping your body form a thinner, more pliable capsule. Early clinical data shows capsular contracture rates as low as 0.5% at three years, among the lowest reported for any brand. (For context, traditional smooth implants average 3-10% over similar timelines.) Reducing that risk can improve comfort and preserve your final shape.

Do Motiva implants go over or under the muscle?

Either! Motiva’s advanced gel technology makes them suitable for:

  • Submuscular placement (under-the-muscle) – Traditional option; adds extra coverage and generally a more natural look and feel.
  • Subfascial placement (over-the-muscle but under the fascia) – A preservation technique that keeps the pectoralis muscle fully intact, but may make implants more obvious for some patients. Motiva’s soft gel and low rippling risk make this position increasingly popular.

During your consultation, Dr. Reedy will help you decide on the best placement for your unique anatomy and goals.

What projection options are available with Motiva?

Both Round and Ergonomix implants currently come in three projection profiles:

  • Mini – lowest profile, subtle forward projection
  • Demi – moderate-low
  • Full – moderate-high

These levels let Dr. Reedy fine-tune your silhouette, matching chest width and aesthetic goals without relying solely on volume.

Should I swap out my older implants for Motiva?

Whether you should swap out your current implants for new Motiva implants will depend on your situation:

  • Are you happy with your implants? If they’re intact, feel comfortable, and look great, there’s no medical rush to change them.
  • Dealing with capsular contracture, rippling, or animation deformity? Upgrading to Motiva for breast implant revision can correct those issues and potentially lower the recurrence risk.
  • Seeking a softer, more natural feel or better upper-pole fullness? Motiva’s dynamic gel and expanded projection range could be worth the switch.

If you’re curious, schedule an in-office evaluation so Dr. Reedy can assess shell integrity, capsule thickness, and your aesthetic goals. We’ll give you an honest recommendation—no pressure.
